How to make claim
In the event of an accident or an unexpected health problem that is covered by your policy, the insured can claim the benefits of their policy by following the process below:
1. Contact the CIGNA Customer Service Center on the telephone number as provided to you in your insurance policy.
2. CIGNA Customer Service will check your policy status and coverage conditions.
3. If the policy is still going, CIGNA Customer Service will ask you to supply the supporting documents to start the claims process.
4. We ask that you prepare all the documents and send them to the Company at the address given in your policy.
This will ensure that we can process your claim as fast as possible.
5. CIGNA will determine the compensation payment and inform you within 20 days after the receipt of the completed documents.
Documents to be submitted for initial claim process
- Claim documents for Personal Accident Plan (Accidental Death)
- Claim documents for Personal Accident Plan (Medical expenses claim from accident)
- Claim documents for Hospital Income Plan in case of hospitalization due to sickness or accident
- Claim documents for Dental Care Plan (Dental Family Package)
- Claim documents for Terminal Illness or Critical Illness

1. Personal Accident Plan policy
2. Claimer dictum and Claim Form (Please click here to download Form)
3. Medical Certificate with the Company Form (Please click here to download Form)
4. Original Death Certificate, copy of death certificate must be certified by local registrar
5. Copy of the ID Card or Government official ID of insured and beneficiary certified by beneficiary
6. Census copy of beneficiary certified by beneficiary
7. Census copy with insured name and local registrar stamped “death” from census and certified true copy
8. Policy report from Royal Thai Police and certified by police officer
9. Death certification from hospital (if applicable)

Payment method
Customer can specify payment method in company claim form by:
1. Direct credit to bank account.
- Please attach claimant’s book bank copy.
2. Cheque
